It simply means a premium user downloaded your file.

You earn on premium sales when a visitor buys a premium that came to the site for one of your files and decided to buy a premium. Then you get percentage of the sale.

When a user comes to the site and downloads a file WITHOUT buying a premium then you will make money based on the PPD table.
